Care Management Support Coordinator II – LTSS Medicaid

About the role

Centene is seeking a Care Management Support Coordinator II to provide administrative care‑management support for members receiving Long‑Term Services and Supports (LTSS) under Medicaid. The role involves outreach, call handling, scheduling, and serving as a primary point of contact for members, providers, and internal staff.

Key responsibilities

• Perform outbound outreach to members to guide them through care‑plan steps, answer questions, and provide education.
• Handle inbound calls, schedule services, and connect members to health‑plan and community resources.
• Maintain non‑clinical member records in compliance with state and regulatory guidelines.
• Support member onboarding by sending welcome letters, correspondence, and educational materials.
• Provide front‑line assistance on member and provider inquiries, explaining care‑plan procedures and protocols.
• Identify and refer members to resources that address Social Determinants of Health (SDOH).
• Assist with processing healthcare authorizations and coordinating services.
• Perform other duties as assigned while adhering to company policies.

Required profile

• High School diploma or GED.
• 1–2 years of related experience.
• Preferred: 2+ years supporting adults 21+ receiving LTSS.
• Preferred: 2+ years of outbound member or provider outreach.
• Preferred: 2+ years processing healthcare/insurance authorizations.
• Detail‑oriented, adaptable, hands‑on learner.
• Proficient with web‑based and CMS systems.
