Telephonic Nurse Case Manager (Full‑time)

About the role

Paradigm is seeking a full‑time, benefitted Telephonic Nurse Case Manager to provide remote medical case management for injured individuals, many of whom have industrial injuries. The role works Monday‑Friday within the Eastern time zone, coordinating care, exploring vocational options, and helping clients return to optimal function.

Key responsibilities

• Deliver remote case management services to injured workers, ensuring timely coordination of medical services.
• Assess recovery progress regularly and adjust care plans as needed.
• Collaborate with claim examiners, employers, attorneys, and medical providers to facilitate optimal outcomes.
• Explore and recommend vocational options, supporting return‑to‑work or alternate employment.
• Maintain accurate documentation and adhere to regulatory and certification standards.

Required profile

• Active, unencumbered compact RN license.
• National certification such as CCM, CRRN, CDMS, or COHN.
• Experience in case management, preferably with industrial injury populations.
• Strong communication skills for interaction with multiple stakeholders.

Required skills

• Valid compact RN licensure.
• Relevant national case‑management certification (CCM, CRRN, CDMS, COHN).
