Physical Therapist – Home Healthcare

About the role

The Physical Therapist delivers primary rehabilitative care to 25‑35 patients in their homes, addressing physical, emotional, developmental, psychosocial, and educational needs. The role involves coordinating multidisciplinary services, supervising PTAs and Home Health Aides, and ensuring high‑quality outcomes throughout each care episode.

Key responsibilities

• Assess, develop, and manage individualized care plans in collaboration with the care team and physicians.
• Perform routine and complex therapy procedures, adjusting visit schedules based on patient needs.
• Coordinate services across disciplines, revise care plans, and monitor goal achievement.
• Educate patients and caregivers to promote self‑care and independence.
• Communicate status changes to physicians and participate in team conferences.
• Ensure regulatory compliance, complete accurate documentation, and engage in performance‑improvement activities.
• Contribute to staff education, meet productivity standards, and fulfill weekend and call rotation duties.

Required profile

• Graduate of an accredited physical therapy program.
• Physical Therapist license, BLS/Instructor certification, valid driver’s license, and private vehicle insurance.
• Minimum 1 year full‑time PT experience; home‑health experience preferred.
• Strong assessment, rehabilitation, leadership, organizational, and communication abilities.

Required skills

• Proficiency with Windows‑based software.
• Knowledge of OASIS (Outcome and Assessment Information Set) processes.
