Care Manager (RN)

About the role

The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with primarily physical health needs. The role focuses on creating personalized care plans, educating members and families, and ensuring high‑quality, cost‑effective outcomes.

Key responsibilities

• Evaluate member needs, barriers, and social determinants of health, then recommend and facilitate appropriate care plans.
• Develop and maintain ongoing care/service plans, collaborating with providers, specialists, and community resources.
• Identify problems or barriers to care and implement targeted care‑management interventions.
• Coordinate communication among members, families, caregivers, and the care‑provider team to ensure person‑centered services.
• Provide ongoing monitoring, follow‑up, and plan adjustments based on member status and progress.
• Offer resource support for employment, housing, independent living, and other social services.
• Document all activities to comply with state, federal, and payer regulations.

Required profile

• Registered Nurse licensure (state or compact) required.
• Bachelor’s degree in Nursing and 2–4 years of related experience, including obstetric (OB) experience.
• Must reside in Georgia.
