Care Manager – Remote (Georgia)

About the role

The Care Manager will provide comprehensive, high‑quality care coordination for members with mental health and substance‑use needs. Working remotely from Georgia, you will develop personalized care plans, educate members and families, and ensure access to appropriate behavioral‑health services.

Key responsibilities

• Assess member needs via phone, digital tools, or home visits and create individualized care plans.
• Identify and connect members with mental‑health providers, specialists, and community resources.
• Monitor care plans, track outcomes, and adjust recommendations based on clinical changes.
• Document all activities to comply with state, federal, and payer regulations.

Required profile

• Master’s degree in Behavioral Health, Social Work, or Nursing.
• Active RN license or Licensed Master’s Behavioral Health credential (e.g., LCSW, LMSW, LMFT, LMHC, LPC).
• 2–4 years of related experience in behavioral‑health care management.
• Strong time‑management, assessment, and communication skills; comfortable using computers and telephonic outreach.
